Resident permit of a family member of EU citizen

Documents needed to be submitted to the application for issuing residence permit for family member of EU citizen

• two photographs of size 3 x 3,5 cm
• valid travel document
• document which confirms the existence of family relationship with gurantee (certificate of birth, certificate od marriage)
• confirmation of registrating the guarante

In case of family member of the EU citizen who

• is a child under the age of 21, unprovided child or such child of his/her spouse, he/she shall submit documents which confirm this fact.
• is a dependent relative in lineal consanguinity or such a person of his/her spouse, he/she shall submit documents which confirm this fact.

In case of family member of the EU citizen who

• is any other family member who falls outside of Art. 2(5)(a)-(c) of Act on residence of Aliens he/she shall submit the reliable document which confirms that he/she is a person dependent on guarantee.

In case of family member of the EU citizen who

• is any other family member who falls outside of Art. 2(5)(a)-(c) of Act on residence of Aliens and is a member of EU citizen´s household, he/she shall submit the reliable document which confirms this fact.

In case of family member of the EU citizen who

• is any other family member who falls outside of Art. 2(5)(a)-(c) of Act on residence of Aliens and due to severe health condition is dependent on EU citizen´s care, he/she shall submit the reliable document which confirms this fact.

In case of family member of the EU citizen who

• is a partner of EU citizen with whom has a permanent, duly certified relationship (e.g. registered partner) he/she shall submit the reliable document which confirms this fact.

In case of family member of the EU citizen who

• is third country national enjoying the right of residence of the family member of the EU citizen in the member state in which the Slovak republic national is the EU citizen with whom the third country national returns or joins him/her to reside back in the Slovak republic territory and fulfils some of the conditions specified in Art. (2)(5) (a) to (g) in relation to the Slovak republic national, he/she shall submit the residence permit of family member of EU citizen issued by the member state.
